Understanding premature ejaculation
Premature ejaculation (PE) means regularly ejaculating sooner than you or your partner would like during sex, with little sense of control. It is one of the most common sexual concerns men report, and it affects men of all ages. Occasional episodes are completely normal. It is worth seeking help when it happens most of the time and is causing frustration, avoidance or strain in a relationship.
Causes are usually a mix of factors. Anxiety, stress and early sexual experiences play a large role, and physical factors such as heightened sensitivity, prostate or thyroid issues can contribute. Some men have experienced it since their first sexual encounters, while for others it develops later. Both patterns respond to treatment, and speaking to a clinician is the first step.

When to see a doctor
PE itself is rarely a sign of anything serious, but see your GP promptly if you notice any of the following, as they need in-person assessment.
- Pain on ejaculation, or blood in your semen or urine
- Urinary symptoms such as difficulty passing urine or needing to go often
- Premature ejaculation that starts suddenly alongside erection difficulties
- Low mood or anxiety that is affecting your daily life
If any of these apply, your GP can check for underlying causes such as prostate or thyroid issues. For urgent advice that is not an emergency, call NHS 111.

Treatment approaches
Several evidence-based approaches exist for premature ejaculation, and they are often combined. Behavioural techniques such as the stop-start and squeeze methods, along with pelvic floor exercises, help many men build control over time. Topical products applied shortly before sex reduce sensitivity and are available without a prescription, subject to pharmacist approval. There is also a prescription tablet licensed specifically for premature ejaculation, taken a few hours before sex. It is not suitable for everyone, which is one reason a clinical review matters.
Where anxiety or relationship factors are involved, talking therapies and psychosexual counselling can make a real difference, on their own or alongside treatment. If you also experience erection difficulties, tell your prescriber, because treating that often improves both. Your prescriber will consider your overall health before recommending any option.
